Country singer Glen Campbell has released a heartbreaking final song and video.

I'm Not Gonna Miss You, recorded in 2013 with producer Julian Raymond, is the final song he will record since being diagnosed with Alzheimer's disease in 2011, according to CNN.

"I'm still here but yet I'm gone / I don't play guitar or sing my songs," the song begins. Campbell, 78, goes on to sing about his wife, Kim, saying "You're the last person I will love / you're the last face I will recall," as clips of the two through the years play in the music video. The career-spanning video also flashes through performances, family home videos and doctors discussing brain scans with him.

The Country Music Hall of Famer, best known for songs Rhinestone Cowboy and Wichita Lineman, was admitted to a special care facility in Nashville in April.

Speaking with USA TODAY last year, Campbell remained optimistic about his condition. "There's nothing bothering me," he said. "I'm actually getting along with it. I told it to get out or something. I don't even know it's there.

"I always wanted to sing songs from the heart," he added later. "I still do."
